In this form, you will provide information about your educational background, your work experience, your leadership experience and you ideas and beliefs about leadership in general.
Participant’s Leadership Competency Assessment – Pre Course
In this form prior to the start of the course you will rate a variety of competencies on how important each one is to becoming and effective leader. The assessment will be repeated at the conclusion of the course, you will repeat the assessment.
Informed Consent Form
As stated in the course description, this course also serves as a data collection tool for a PhD study being conducted by the course instructor. As such, each participant must complete the “Informed Consent Form” to be eligible to participate in the research project. Participation is voluntary, and participants may withdraw their consent at any time, by informing the researcher.
